LENS REPLACEMENT

Your replacement form

Fill this in, print it, and pop it in the box with your sunglasses. It tells us whose frames we've got and what to fit — without it, your frames sit on a bench while we work out who they belong to.

Before you send anything:

1. Order your replacement lenses — polycarbonate, $25 or glass, $100.

2. Fill in the form below and print it.

3. Wrap your frames well — the microfiber pouch inside a sturdy box works. Frames damaged in transit can't be fitted.

4. Mail them to us at the address on the form. Postage to us is at your own cost; we'll cover sending them back.

Tip: use your browser's print option (Ctrl+P, or Cmd+P on a Mac). Only the form below prints — everything else on this page is left off.

Lens replacement is available on non-prescription Island Optics sunglasses, once per purchase, within 12 months of the original order. Glass lenses are available on the frames we offer with a glass option. Questions? orders@islandoptics.com or (800) 786-8721.

Can't print? No problem — write the same details on a sheet of paper and include that instead. We just need to know whose frames we're holding.
